India's Ageing Population Is Booming — Can Regenerative Medicine Ease the Joint Pain Crisis?

India is in the middle of an unprecedented demographic shift. The country's population of citizens over 60 is projected to double by 2050 according to the United Nations, and by 2050, the number of Indians aged 60 and above is expected to reach roughly 347 million, up from 149 million in 2022. Put simply: one in every five people in India will be an elderly citizen by 2050. Longer lifespans are, in many ways, a healthcare success story. But they come with a quieter consequence - a sharp rise in age-related musculoskeletal disorders, from osteoarthritis to osteoporosis, that are steadily eroding mobility and independence for millions.

We spoke to Dr Geetika Jassal, Medical Spokesperson, Cryoviva Life Sciences, who explained why this shift demands a fundamental rethink in how India approaches joint health, and why regenerative medicine is emerging as a serious part of that conversation.

A Global Burden, With an Indian Accent

Joint pain is not a niche complaint. Globally, an estimated 1.71 billion people have health conditions that would benefit from rehabilitation for musculoskeletal conditions, and musculoskeletal conditions remain the leading contributor to disability worldwide, with low back pain the single leading cause of disability in 160 countries. These aren't abstract statistics - they translate into millions of people who can no longer climb a flight of stairs without pain, or sit through a family function without needing to shift position every few minutes.

In India, this burden carries a distinct local signature. "An ageing population is colliding with rising obesity rates and increasingly sedentary lifestyles, a combination that accelerates wear on joints far earlier than it used to. What was once considered an "old age problem" is now showing up in patients in their forties and fifties, layered on top of a growing elderly population that is living longer, but not always more comfortably," said Dr Jassal.

Why Symptom Management Alone Isn't Enough Anymore

For decades, the toolkit for joint disorders has stayed fairly consistent: pain medication, physiotherapy, intra-articular steroid injections, and, when things progress far enough, joint replacement surgery. These remain essential, particularly for advanced disease - nobody is suggesting they disappear.

But there's a growing gap in the middle of the treatment journey. "Many patients aren't quite ready for surgery, yet conventional management barely holds the line on pain and stiffness. This is the space regenerative medicine is trying to occupy - not as a replacement for existing care, but as an option that may help preserve joint function, ease symptoms, and potentially slow disease progression before a scalpel becomes necessary," added Dr Jassal.

At its core, regenerative medicine works with biological therapies and tissue engineering approaches that aim to support the body's own healing mechanisms, rather than simply masking pain. It's a meaningful shift in framing, from managing decline to attempting repair, though it's worth being clear-eyed that this field is still evolving, and several of its therapies remain under active investigation rather than settled, routine care.

From Laboratory to Clinic: How Far Has Regenerative Medicine Come?

Regenerative medicine has moved well past being a theoretical idea confined to research papers. "Worldwide, substantial investment is flowing into translational research, biotechnology, and clinical trials evaluating therapies for degenerative musculoskeletal disorders. Some of these approaches have shown encouraging outcomes in carefully selected patients; a meaningful signal, even if it isn't yet proof of universal benefit," shared Dr Jassal.

That caveat matters. Many of these therapies still need robust, long-term clinical evidence before they can be considered part of routine standard care. This is a field that rewards patience and scientific rigour over hype, something that's especially important in a market where "regenerative" and "stem cell" therapies are sometimes marketed ahead of the evidence that should support them.

For India specifically, the timing is notable. The country's healthcare system is expanding rapidly, and demand for orthopaedic services is climbing alongside it. Within that growth, evidence-based regenerative medicine could become a genuine complement to conventional treatment; not a replacement, but an additional option that, for select patients, may help reduce disability, improve mobility, and delay the need for more invasive surgical intervention.

Building a Healthcare System That's Ready for an Older India

The bigger question isn't just what regenerative medicine can do; it's whether India's healthcare system is structured to make the most of it. "As the country prepares for a demographic where nearly one in five people will be over 60, the shift needs to go beyond individual treatments. It calls for a broader move from reactive disease management to proactive strategies built around prevention, early diagnosis, rehabilitation, and healthy ageing," said Dr Jassal.

That means real investment in musculoskeletal health as a category - preventive orthopaedics, earlier diagnosis pathways, stronger rehabilitation infrastructure, continued clinical research, and access to regenerative therapies that have been scientifically validated rather than simply marketed.

"The future of orthopaedic care in India is unlikely to be defined solely by joint replacement numbers. Increasingly, the focus is shifting towards joint preservation, repair, and restoration wherever that's genuinely possible," concluded Dr Jassal.
