Scroll through social media for long enough, and you'll find a 20-something explaining why they've already booked their first Botox appointment - not to fix a wrinkle, but to stop one from ever forming. It's a trend backed by real numbers: the American Society of Plastic Surgeons has recorded a 71 percent rise in neuromodulator injections among people aged 20 to 29 between 2019 and 2022, part of what dermatologists now call "prejuvenation," a shift documented in the Aesthetic Surgery Journal Open Forum as the biggest change in cosmetic dermatology in two decades.
But does starting early actually mean starting smart? Dr Noopur Jain, Founder and Consultant Dermatologist at Skinzest, Gurgaon, says the answer has less to do with a birthday and more to do with why someone is walking into the clinic in the first place.
Preventive Aesthetics Is A Mindset, Not A Start Date
Preventive aesthetics is often misunderstood as a countdown - a specific age by which treatments should begin. Dr Jain pushes back on that framing.
"In my practice, I think starting anti-ageing treatments early is possible. This happens when people start treatments because of age, social media trends, or fear of wrinkles. A 25-year-old does not automatically need Botox or fillers, and a person in their 40s does not always need procedures. Aesthetic treatments should be based on the individual, not on the birthday number," said Dr Jain.
Why Skincare Should Come Before Any Syringe
For most patients, Dr Jain's starting point isn't a treatment room at all - it's the basics that tend to get skipped in the rush towards procedures.
For patients, preventive aesthetics should start with the basics: consistent sun protection, proper skincare, a healthy lifestyle, and fixing specific concerns like pigmentation, acne, or early changes in texture. Good skincare and protection from environmental damage can go far in maintaining skin quality.
When Procedures Make Sense
That doesn't mean injectables and lasers have no place in a preventive approach - Dr Jain says they simply need to be earned through a proper assessment, not a trend cycle.
"When it comes to procedures, I lean toward a conservative, personalised stance. Treatments such as anti‑wrinkle injections, fillers, lasers and other collagen‑stimulating methods have their role. They should only be considered when there is a real need and after reviewing facial structure, skin quality, muscle movement and expectations," advised Dr Jain.
The purpose of aesthetics is not to change someone's look or freeze facial expressions. If a treatment fits, the aim is to keep features and keep a refreshed, well‑rested look. This matters because much treatment can make a person look less natural, not younger.
The "Less Is More" Philosophy
Dr Jain's approach is guided by a single governing principle, one that runs counter to the "more is better" instinct social media often encourages.
"My philosophy is 'Less Is More'. Preventive aesthetics should lift what is already there, not try to change a person. The right treatment, done at the right time and in the right amount, can be far better than chasing popular procedures," shared Dr Jain.

Dr Jain concluded, "There is no age when a person should start anti‑ageing treatments. Preventive aesthetics works best when it is tailored, evidence‑based, and matches each patient's needs. Starting early can help some. Starting when not needed can backfire. The goal should always be natural skin and graceful ageing, not a chase to look forever young."
