Cough That Lasts More Than Three Weeks: Could It Be Lung Cancer?

A cough is one of the most common symptoms people experience, and in many cases it settles with simple treatment. However, whether you are a smoker or a non-smoker, young or old, a cough lasting for more than two weeks should not be ignored.

We spoke to Dr G Chaitanya Kiran, Consultant - Interventional Pulmonology and Sleep Medicine, Manipal Hospitals, Bhubaneswar, who explains what a lingering cough could be telling you, and when it's time to stop waiting it out.

When a Persistent Cough Becomes a Red Flag

"If a persistent cough is associated with bleeding while coughing, unexplained weight loss, loss of appetite, or fever that does not subside with basic medications, it needs proper medical evaluation," warned Dr Kiran. While these symptoms may be caused by infections or other lung diseases, in the current scenario they should also raise suspicion for lung cancer and require timely investigation.

Lung cancer is already among the most commonly diagnosed cancers in India, ranking fourth overall according to Globocan 2022 data, making awareness of its early warning signs all the more important.

Who Should Be Extra Vigilant

Lung cancer screening and evaluation need greater awareness among the general public, especially those with known risk factors. "These include active smoking, passive smoking, vaping, a family history of cancer, and lung conditions that do not resolve despite appropriate treatment. Early evaluation in such individuals can help identify disease at a stage when treatment options are more effective," said Dr Kiran.

How Lung Cancer Diagnosis Has Evolved

The approach to diagnosing lung cancer has evolved considerably over the years. Today, evaluation ranges from a simple chest X-ray to advanced imaging such as PET-CT scans and molecular marker testing. "Diagnostic methods have also progressed from sputum cytology to image-guided and bronchoscopic lung biopsies, allowing greater accuracy in confirming the diagnosis and planning treatment," added Dr Kiran.

The Pulmonologist's Expanding Role

The role and responsibility of the pulmonologist have also expanded significantly. Modern diagnostic procedures such as bronchoscopy and EBUS-guided lung biopsies have become essential tools in detecting and staging lung cancer. "At the same time, successful diagnosis depends on a multidisciplinary approach involving specialists in radiology, pulmonology, pathology, and nuclear medicine before patients are referred to the oncology team for definitive treatment. This coordinated pathway plays a crucial role in ensuring accurate diagnosis and timely management," explained Dr Kiran.
