Breast Cancer: A Lump Is Not The Only Warning Sign, Says Doctor
Many women check their breasts for lumps but may not think much of a change in the skin or nipple. A patch of redness may be blamed on irritation, a nipple that has started turning inward may go unnoticed, and a change in the shape of one breast may be put down to age or hormonal changes.
These changes are not necessarily signs of cancer. But when something looks or feels different from what is normal for you, it deserves attention, even if there is no lump. We spoke to Dr Venkatachala K, Consultant Surgical Oncologist at HOSMAT Hospital, who shared warning signs of breast cancer other than a lump.
When the skin starts to change
Breast cancer can sometimes show up as a change in the skin rather than a distinct lump. The skin may look puckered or dimpled, almost as though it is being pulled inward.
"In some cases, it becomes thicker, swollen or develops an orange-peel-like appearance, with visible pores and a rough texture. Skin nodules like pimples on the breast might be cancer deposits as an unnoticed disease progresses," said Dr Venkatachala.
Redness, darkening, scaling or a persistent rash can also occur. These changes may resemble an infection or a skin condition, which is why they can be easy to dismiss. A rare form called inflammatory breast cancer can cause rapid swelling, warmth and skin changes without an obvious lump.
A rash that does not settle, particularly around the nipple, should not simply be treated repeatedly with creams without finding out what is causing it.
Pay attention to the nipples
A nipple that has always been inverted is usually not a cause for concern. A nipple that recently begins pulling inward, changes direction or looks different from before is another matter and needs examination.
"Discharge is worth noticing as well. Fluid from the nipple can have several causes, but spontaneous discharge, particularly if it is bloody or comes from only one breast, should be checked," added Dr Venkatachala.
Persistent crusting, scaling or a sore on the nipple can occasionally be a sign of Paget disease of the breast, a rare form of breast cancer affecting the nipple area. It may look like eczema, so a persistent change should not be assumed to be a simple skin problem.
Changes in shape, swelling or pain
One breast becoming noticeably larger or swollen, or a new difference developing between the two breasts, can be another warning sign. Swelling in the armpit or above the collarbone also needs assessment, as it may be an enlarged cancerous lymph node.
Breast pain on its own is not usually a sign of cancer. It might happen in advanced stages. Hormonal changes, cysts and other benign conditions are much more common explanations. However, pain that persists in one area, particularly alongside another change, should not be ignored.
What should you do if you notice a change?
The first step is to see a doctor rather than wait for a lump to appear. An examination, followed by an ultrasound or mammogram depending on the person's age and symptoms, may be recommended. If an abnormal area is found, a biopsy may be needed to establish the diagnosis.
Most breast changes turn out not to be cancer. That is reassuring, but it is not a reason to ignore them. Even a recent normal screening test should not be used to dismiss a new symptom.
Breast self-examination
Knowing what is normal for your breasts makes it easier to notice a change, whether while bathing, getting dressed or simply looking in the mirror. A lump is an important warning sign, but it is not the only one. If a change is new, persistent or unusual for you, get it checked rather than waiting for it to become more obvious.
Breast awareness is not just about finding lumps; it is about knowing your body well enough to recognise when something changes, and seeking medical advice without delay.
