World Parkinson’s Day 2026: How to Tell If a Tremor Is Parkinson’s or Something Else
A slight shake in the hands can be easy to brush off or quietly worry about. For many, it raises an immediate question: Is this something serious like Parkinson's, or just a temporary response to stress or fatigue? The truth is, not every tremor points to a neurological disorder. Often, it's the subtle details, when the shaking happens, what you're doing at the time, even how your handwriting looks, that can tell a very different story.
We spoke to Dr Kaustubh Mahajan, Consultant, Neurology, PD Hinduja Hospital and Medical Research Centre, Mahim, who explained how subtle differences can help distinguish between Parkinson's and more common, non-serious causes of shaking.
Not All Tremors Mean Parkinson's
All tremors or shaking are not necessarily due to Parkinson's disease. "Many people experience shaking at some point, but the pattern and timing of the tremor can help in understanding whether it is related to Parkinson's or due to other, more common causes. Careful observation of when the tremor appears and how it behaves is important in differentiating between the two," explained Dr Mahajan.
What Are Rest Tremors?
Tremors that are more noticeable at rest and tend to improve with action are known as rest tremors. These are typically seen in Parkinson's disease. "A person may notice shaking in their hands when they are sitting quietly or not using their hands. As soon as they begin to use their hands for any activity, the tremor often becomes less noticeable. This pattern, where the tremor is present at rest and improves with movement, is commonly associated with Parkinson's tremors," said Dr Mahajan.
Understanding Action Tremors
"In contrast, the opposite pattern is also frequently seen. Tremors that are more noticeable during action and improve with rest are called action tremors. These are more commonly observed in day-to-day practice and are usually not related to Parkinson's disease," added Dr Mahajan. Action tremors can be physiological, may occur due to anxiety, can be seen in essential tremor, or may be induced by certain drugs. These tremors tend to appear when a person is trying to perform an activity.
What Your Handwriting Can Reveal
Another simple way of differentiating between these tremors is by asking the person to write. Handwriting can provide useful clues. In individuals with action tremors, the handwriting is usually bigger than normal and appears shaky.
Why Early Differentiation Matters
On the other hand, in individuals with Parkinson's or rest tremors, the handwriting is much smaller than usual and does not appear shaky.
"Thus, by observing when the tremor occurs and by looking at handwriting patterns, one can differentiate between Parkinson's tremors and other types of tremors. Early recognition of these differences is important, as timely evaluation and early intervention can help in better management and improved outcomes," concluded Dr Mahajan.
