Is Willpower Really The Problem? The Gut-Brain Science Behind Sugar Cravings

For years, sugar cravings have been filed under a single, unforgiving explanation: a lack of willpower. That story is getting harder to hold onto. A growing body of research shows that appetite has as much to do with the gut as it does with self-control.

We spoke to Shafiulla Hirehal Nuruddin, Founder and Managing Director of Greenspace Herbs, a Bengaluru-based nutraceutical company, about what the emerging science of the gut-brain axis means for how we understand cravings, and where compounds like berberine fit into that picture.

The Conversation Between Gut And Brain

"The gut-brain axis refers to the two-way communication between the gut and the brain, carried out through hormones, nerves and microbial metabolites. Signals like GLP-1 and PYY, released in the gut, can influence satiety and how much a person eats," said Nuruddin.

The microbiome is very much part of that conversation. Microbes in the gut produce metabolites that can shape appetite signalling, energy metabolism and inflammation. A recent review has pointed to short-chain fatty acids, bile acids, amino acid metabolites and tryptophan derivatives as key contributors to this gut-brain signalling.

Where Berberine Fits In

Berberine, a compound derived from plants, has drawn growing attention for its effects on glucose and metabolic health. A 2024 systematic review and meta-analysis of 50 randomised trials involving 4,150 people found that berberine, whether used alone or alongside glucose-lowering medicines, was associated with improvements in blood glucose, HbA1c, insulin resistance and lipid measures. The study's authors called for further research into its mechanisms and its role alongside conventional treatment.

Changing The Signals, Not Switching Off Cravings

The experimental picture is intriguing, if early. In a study of people with mild metabolic disturbances linked to olanzapine treatment, 12 weeks of berberine supplementation shifted the abundance of several bacterial groups while also improving glucose, insulin resistance and triglyceride measures.

Other research suggests berberine can influence bacteria associated with short-chain fatty acid production, as well as pathways linked to GLP-1 signalling. In animal models, these microbiome shifts have come with changes in gut hormones and reduced food intake.

"That doesn't mean berberine wipes out 'sugar-loving' bacteria or flips a switch on cravings. The microbiome doesn't work that way - it's too complex for single-cause explanations. A more accurate way to read the research is that berberine may influence the microbial and metabolic signals that shape how the gut talks to the brain," explained Nuruddin.

The Next Step Is Precision

This distinction matters. Future research needs to move past hunting for a single ingredient that works the same way for everyone. The real question is which compounds affect which microbial communities, metabolites and metabolic pathways - and under what conditions.

"Advanced analytical techniques, phytochemical profiling, computational modelling and clinical research can help map these relationships more precisely," according to Nuruddin.

The goal isn't to replace dietary discipline with a pill or a supplement. It's to understand the biology of appetite well enough to build more targeted, evidence-based approaches to metabolic health.

Changing The Conversation

Nuruddin concluded, "Berberine is still being studied, and the evidence doesn't support the idea that it can effortlessly erase sugar cravings in humans. What it does offer is a compelling research direction - the possibility that managing metabolic health means changing the conversation between our microbes, our metabolism and our brain, across different people, diets and metabolic states."
