Eating Clean but Still Falling Sick? The Real Reasons Lifestyle Diseases Are Rising
"One of the most common things I hear in my clinic these days is confusion. Patients sit across from me and say, "Doctor, I eat clean. I don't eat junk food. I exercise sometimes. Why am I still developing diabetes, blood pressure, or cholesterol issues?" said Dr Sandeep Reddy Koppula, HOD - Internal Medicine, Arete Hospitals, Hyderabad.
These are not people living on fast food. Many are home-cooked-meal eaters, avoid sugar, drink green tea, and read nutrition labels carefully. Yet they're being diagnosed with lifestyle diseases earlier than expected. The frustration is understandable.
What's becoming increasingly clear in medical practice is this: food alone no longer tells the full story. Stress, poor sleep, and gut inflammation are quietly undoing many of the benefits of so-called healthy eating.
Chronic Stress: The Silent Metabolic Disruptor
Chronic stress has become normalised, especially in urban India. Long working hours, financial pressure, constant connectivity, and little mental downtime keep the body in a prolonged state of alert. "When stress hormones like cortisol remain elevated, they interfere with blood sugar control, increase abdominal fat storage, raise blood pressure, and worsen cholesterol patterns. A person may be eating the "right" foods, but hormonally, the body behaves as if it is under threat," said Dr Koppula.
"I often explain to patients that stress doesn't just affect the mind, it directly alters metabolism. In fact, I see many cases where stress management makes a bigger difference to sugar and blood pressure levels than dietary changes alone," he added.
Poor Sleep: The Overlooked Health Risk
Sleep is the second major piece that gets overlooked. Many people underestimate how damaging poor sleep can be. Late nights, screen exposure, irregular sleep schedules, and constant mental stimulation prevent the body from fully resting. Even six hours of broken sleep, night after night, is enough to disrupt insulin sensitivity and appetite-regulating hormones.
"When sleep is compromised, the body becomes more prone to inflammation and cravings. People may eat well during the day, but the hormonal damage from inadequate sleep quietly builds up. Over time, this contributes to weight gain, rising sugars, fatigue, and poor concentration," explained Dr Koppula.
Gut Health and Inflammation: The Missing Link
Then there's the gut, an area we are only beginning to fully appreciate. The gut isn't just about digestion; it plays a major role in immunity, inflammation, and metabolic health. Stress, erratic meal timings, frequent antibiotics, and ultra-processed "healthy" foods can disrupt the gut microbiome.
Many packaged foods marketed as healthy, protein bars, sugar-free snacks, and diet drinks contain additives that irritate the gut lining. "A chronically inflamed gut sends inflammatory signals throughout the body, worsening insulin resistance and contributing to fatigue, bloating, and even mood changes. Patients are often surprised when gut health becomes part of a conversation about diabetes or cholesterol, but the connection is very real," said Dr Koppula.
The Problem With Extreme Health Routines
"Another issue I see frequently is the "all-or-nothing" approach to health. People eat extremely clean for a few days, then crash from exhaustion or stress and fall off routine. The body thrives on consistency, not perfection. Skipped meals, long fasting hours combined with high stress, and extreme diets can actually backfire, increasing hormonal strain," shared Dr Koppula.
What Actually Helps?
The answer is less dramatic than most people expect.
Regular meal timings, adequate protein, and simple home-cooked food still matter, but they must be supported by sleep and stress regulation. Even 20-30 minutes of daily physical activity helps regulate blood sugar and stress hormones. Equally important is downtime without screens, something many adults rarely allow themselves.
Gut health improves with regular meals, fibre-rich foods, fermented foods when tolerated, and reducing unnecessary medications or supplements. There is no single "superfood" that can undo chronic stress or sleep deprivation.
Bottomline
Dr Koppula concluded, "As a physician, my biggest message to patients is this: lifestyle diseases today are not just diseases of excess food. They are diseases of overload, mental, emotional, and physiological. Eating healthy is important, but it cannot work in isolation."
"When stress is addressed, sleep improves, and the gut is supported, many patients see better control of their health markers, sometimes with fewer medications. Health today requires a broader view, and the sooner we recognise the real villains, the better chance we have of staying well."
