1. Lifestyle Issues: The doctor can analyse your sperm count or check if your fallopian tubes are clear. But your doctor cannot monitor your lifestyle on a daily basis. If you are leading an unhealthy lifestyle against your doctor's advise then you know the cause of your infertility very well. Late nights, stress and unhealthy food get together to keep that positive pregnancy test away from you.
2. Age Itself An Explanation: Sometimes women in an advanced age think that their infertility is unexplained. However, age itself can be an explanation for not being able to conceive. The egg that matures every month in your uterus is not 'formed' monthly. You have all the eggs in your ovary when you are born. So the older you get, the older your eggs get. Your eggs might not support fertilisation after a certain age.
3. Extra Territorial Factors: You both may be in fine reproductive health and doing everything right but as long as the environment around you does not support it, pregnancy can evade you. Factors like eating the right kind of food or having the right body temperature may be minor issues but when pregnancy is hard sought, they too have to be taken into account. For example, if the climate you are in is very cold then you have to eat food that generates heat in your body. These nitty-gtties might escape your doctor's attention but let them not escape yours.
4. Fertility Treatments: Sometimes the reason why you can't get pregnant may be identified and the doctor my suggest an IVF (In Vitro Fertilisation) for you. However, it is not necessary that IVF will work for everyone. It is possible that your body does not respond to fertility treatments at all. So then you have nothing to do but wait for technology to catch up with you.
