Although lung cancer is often associated with smoking, experts say there's an important reality that many people are unaware of-having never smoked does not completely eliminate the risk.

On World Lung Cancer Day 2026, understanding this lesser-known aspect of the disease is more important than ever. Dr. Sarath Chandra Bhrungi, Consultant - Interventional Pulmonologist, Gleneagles AWARE Hospital (A Fortis Healthcare Network Hospital), LB Nagar, Hyderabad, explains why lung cancer can occur in non-smokers, the warning signs that shouldn't be ignored, and why early diagnosis can make all the difference.

Never Smoked? You Can Still Get Lung Cancer

Dr. Bhrungi says, "In my practice, some of the hardest conversations I have are with patients who have never touched a cigarette and are now hearing 'lung cancer' for the first time. Most of them say the same thing: 'But I never smoked.'

We still think of lung cancer as a 'smoker's disease.' The truth is, it isn't. About 1 in 5 people who die of lung cancer in the US have never smoked. If we counted non-smoker lung cancer separately, it would still be among the top 10 cancer killers in the world."

How Can Non-Smokers Develop Lung Cancer?

Dr. Bhrungi explains that smoking is not the only cause of lung cancer. Several environmental, occupational, and biological factors can contribute to the disease.

1. Radon Gas At Home

Radon is a radioactive gas you can't see or smell. It comes up from soil and rock and seeps into houses through cracks in the floor. Spend years breathing it in, and it becomes the #1 cause of lung cancer in non-smokers.

2. Secondhand Smoke

Living with a smoker, or working in a place with regular smoke exposure, puts the same cancer-causing chemicals into your lungs.

3. Workplace Exposure

Construction workers, factory staff, mechanics-people who breathe in asbestos, silica dust, diesel fumes, arsenic, or chromium without proper masks are at higher risk.

4. Air Pollution

Long-term exposure to heavy air pollution, especially PM2.5, causes constant inflammation in the lungs. Over years, that inflammation can lead to DNA changes and cancer.

5. Genetic Changes

Many non-smokers who get lung cancer have a type called adenocarcinoma. It's often linked to specific gene changes like EGFR or ALK. This has nothing to do with smoke and everything to do with biology.

6. Previous Chest Radiation

If you've had radiation to the chest for lymphoma, breast cancer, or other conditions, your risk goes up years later.
